Sinai Celebrates begins on September 1, 2009 at 12:01 am Central Time and ends December 31, 2009 at 11:59 pm Central Time. During this time, Sinai invites you to tell us your happy memories of Sinai, or share a photo of a baby born at Sinai or other Sinai photo, by clicking Submit, providing information about yourself, and entering your story headline and content limited to 150 words or less, or submitting your photo by attaching an image limited to 1 MB or less. By submitting your stories and photos, you agree that Sinai may publish or reprint them, together with your name, including at www.sinaicelebrates.org.
NO PURCHASE NECESSARY. Sinai will hold random drawings of the names of all individuals making submissions and other entries (see below) for four (4) $50 gift certificates from Sinai’s online store, www.cafepress.com. Drawings for one (1) gift certificate will be held monthly on or about the tenth day of the following month for entries received during the months of September, October, November and December, 2009. Winners will be randomly selected from all eligible entries received by Sinai by 11:59 pm of the last day of each of the four months. Winners’ names will be posted at www.sinaicelebrates.org on or about ten days after each drawing.
1) Eligibility: You must be a legal resident of the U.S. 18 years of age or older to submit a story or photo, or enter into the drawing. This program is void in Puerto Rico, U.S. territories and possessions, outside of the United States, and where prohibited or restricted by law.
2) How To Enter: Persons who meet the eligibility requirements and have read and agreed to these Official Rules and Terms of Use may enter the drawing online by going to www.sinaicelebrates.org and either by (a) providing information about themselves when they submit a story or photo, or (b) clicking on Enter to Win and providing information about themselves. You must provide your first and last name, email address and street address to be eligible to win. You also may provide your phone number. Limit one entry per person. If more than one entry per person is received, Sinai reserves the right to void all entries. Incomplete or illegible entries will not be accepted. Sinai is not responsible for incorrect or inaccurate transcription of entry information, or for any human or computer error, technical malfunctions, lost/delayed data transmission, omission, interruption, deletion, defect, line failures, inability to access any website or on-line service, or any other error or malfunction, or late, lost, incomplete, illegible or misdirected entries.
3) Rights in Stories and Photos: By submitting a story or photo, you represent and warrant that (a) the story or photo are your own creations, (b) the story and photo are truthful and accurate, (c) you own all rights, including copyright, to the story or photo, or hold all necessary releases from third parties concerning the story or photo to enable submission to and use by Sinai and its affiliates, and (d) that your submission does not infringe any other person's or entity's rights. If other persons are shown in the photo, you may be required to provide signed releases from such persons or, if under 18 years old, from the parent or legal guardian of such persons. You irrevocably license Sinai and its affiliates the right to publish, display, reprint and reuse the story and photo, and to edit, modify, revise or rearrange the photo or story, in all media throughout the world in perpetuity without compensation or further consent from you, or to not use, publish or display, or remove or repost the story and photo, all in its sole discretion. Sinai will not return acknowledge receipt or return any submissions or entries. Sinai may use your name in connection with publication of the story or photo.
4) Content limitation: You agree not to submit any story or photo that is indecent, obscene, offensive, false, illegal, not suitable for a general audience, outside the intent of the program, or does not comply with these Official Rules as determined by Sinai in its sole discretion. Any such story or photo may be rejected, edited or removed by Sinai and the submitter may be ineligible for the drawings.
5) Waiver and Release: By entering, entrants consent and agree to these Official Rules, and release, discharge, indemnify and hold harmless Sinai and its corporate affiliates from any and all liability or claims concerning use, publication, modification and distribution of the story, photo, submission, your likenesses and statements, and the conduct of the drawings. Acceptance of any prize constitutes consent to use winner’s name, likeness, statements, submissions and biographical data for advertising/publicity purposes in any medium without additional compensation, except where prohibited by law. By accepting any prize, winner agrees that Sinai and its corporate affiliates and their officers, directors, agents and employees will have no liability or responsibility for any injuries, losses or damages of any kind resulting from the submission, entry, drawing, acceptance, possession or use of any prize and to indemnify and hold harmless each of them against any claims of liability arising directly or indirectly from the prizes awarded.
6) Personal Information: By submitting an entry you represent and warrant that you have read and accepted these Official Rules. If you do not uncheck the box asking to be included in future emails or mailings, you understand that you may be contacted by Sinai and its corporate affiliates about updates to the Sinai Celebrates site or Sinai programs, services and events that may be of interest to you.
7) Selection of Winners: A random drawing will be conducted by Sinai or an independent organization on or about the tenth day of the following month for the months of September, October, November and December, 2009, from all eligible entries received by 11:59 pm on the last day of each month. Eligible entries that were not winners in a prior drawing will be included in the next month’s drawing. Odds of winning any prize depend on the number of eligible entries received. By participating, entrants agree that the decisions of Sinai are final and binding in all matters relating to the submission and drawings. Winners will be notified by email and their names will be posted www.sinaicelebrates.org.
8) Prizes: Four (4) $50 gift certificates for Sinai’s online store at www.cafepress.com. The total value of all prizes is $200. Gift certificates are issued by Café Press and are subject to the terms and conditions posted on the Café Press website, see: www.cafepress.com/cp/members/giftcertificates/product_details.aspx#terms. Sinai is not responsible for lost or stolen gift certificates. Gift certificates will be delivered to winners by email. All federal, state and local taxes are the sole responsibility of the winner. Prizes may not be transferred, substituted or redeemed for cash, except that Sinai may substitute prizes of equal or greater value due to prize availability. In the event a winner is unable to accept any prize for any reason, a new winner will be selected following the procedures outlined above. All prizes will be awarded.
9) Administration: Sinai reserves the right in its sole discretion to cancel or modify the drawings. Each entrant is responsible for all on-line charges incurred with any Internet service provider and/or wireless carrier. If Sinai believes, in its sole discretion, that there is any suspected or actual tampering with the submissions, entries or drawings, or if technical difficulties compromise the integrity of the drawings, Sinai reserves the right to void the entries at issue and/or terminate or modify the drawings and/or conduct a random drawing to award the prize using all eligible entries received as of the termination date. If the drawings are terminated due to tampering or technical difficulties prior to its expiration date, notice will be posted at www.sinaicelebrates.org. Any attempt to deliberately damage the content or operation of this program is unlawful and subject to legal action. In the event of a dispute concerning the identity of the entrant, the subscriber for the Internet service provider or wireless carrier for the relevant email address shall be deemed the entrant. Sinai reserves the right in its sole discretion to cancel or modify the drawings. All federal, state and local laws and regulations apply. Void where prohibited or restricted by law.
10) Notice and Procedure for Making Claims of Copyright and Intellectual Property Infringement:
If you believe that your work has been copied in a way that constitutes copyright infringement, or your intellectual property rights have been otherwise violated, please provide Sinai’s Copyright Agent a Notice containing the following information:
1. an electronic or physical signature of the person authorized to act on behalf of the owner of the copyright or other intellectual property interest;
2. a description of the copyrighted work or other intellectual property that you claim has been infringed;
3. a description of where the material that you claim is infringing is located on the site;
4. your address, telephone number, and email address;
5. a statement by you that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law;
6. a statement by you, made under penalty of perjury, that the above information in your Notice is accurate and that you are the copyright or intellectual property owner or authorized to act on the copyright or intellectual property owner’s behalf.

11) List of Winners: The list of winners will be posted at www.sinaicelebrates.org. For a list of winners, send a self addressed stamped envelope with your request to Sinai Health System, 2750 W. 15th Street, Chicago, IL 60608; Attention Marketing Department. Vermont residents may omit return postage.
